Did you? How is this supposed to indicate skill rather than willingness: Draco is too busy being scared of him to enjoy himself properly, right after describing Draco as "evil"?

Yeah, because he was terrified. He's in a room with Bellatrix Lestrange, whose sadism is absolutely unquestionable. If anyone was to enjoy looking at Charity, it would have been her, and yet she ignores her completely because there are more important matters to discuss. Draco is very clearly scared of what he's seeing; this is communicated with great clarity in the words of the chapter (which, in this instance, don't have a Harry-bias and are told from an objective narrator's POV) and pretty much solidly supported throughout the book.

And that's a curse that requires you to have a sadistic streak because you have to mean to hurt your victim in order for it to work.
I don't know how far along in the book are you, so I won't say anything about that, but I don't agree with you that it "requires a sadistic streak". You'll know what I mean near the end. I think.
